CPSM regularly receives concerns from the public, other physicians, pharmacies and labs because they cannot locate the physician they are looking for on the Practitioner Profile Search. This occurs if the name you are practicing under does not match the registered name at CPSM.   

Prescriptions, lab requisitions, and referrals must be made using the name you are registered with at CPSM. The best way to ensure this is done consistently is to practice under the exact name you are registered under.    

Using the same name will avoid confusion when others are confirming your identity for various reasons, including:
